I have worked with an array of populations ranging from prenatal to geriatrics from various racial backgrounds, ethnicity, cultures, religions and creed. My experiences include medical social worker as I worked a various home health agencies, discharge planning, inpatient /outpatient therapist, field based clinical case manager, community based case manager, long term care case manager, both medical and non-medical case manager providing community resources for members.
